Digital Intake Forms
Hello!
I've developed a tool to create digital intake forms which has the ability to generate a QR Code for users/clients to scan. The idea for this website came when I had to make a trip to the walk-in clinic and they had me fill out a bunch of information on pen & paper the old school way. I present the new school way! On this website you can create Intake Forms, specify various fields. Upon creation, you can view the form and share the URL or generate a QR Code which you can print (print interface buggy) and display for clients/customers to scan and fill out. You can then view all submissions. Currently, /dashboard root page and the data points are just for show.
This was built with NextJS 16 and Convex, so everything is realtime! All data included Auth accounts, intake forms, organization data and defaults, submissions, profile pictures and even the "What's New" popup is stored and read from Convex. The original Dashboard layout, UI and colors was generated by Chef, so I translated that into a design file in JSON for future prompts to build off of for design consistency. I plan on adding more features soon like a chat feature on the Thank You/Post Submission page so the client/customer can ask questions to the business/receptionist directly.
